The form must be signed by the patient to allow them to consent to genetic testing. They can still opt to have an appointment with a Genetic Counsellor, if they feel that they require more information before deciding.
The laboratory will not process samples that are accompanied by incomplete or incorrect forms. Please ensure that all of the required information is completed (and legible) before sending your blood sample.
